When to file a report:

In general, an injury report should be completed when an incident occurs requiring use of the first aid kit, or if a player sits out of play following the incident.

Coaches should contact the league safety officer at the end of the practice/game that the injury occurred and direct the parent/guardian to make contact immediately.
